You can download the entire folder of combined PDFs here. Please print, review, and complete all forms prior to your intake appointment. I do have more detailed paperwork than many other clinicians, so please allow yourself that time to thoroughly review and complete these. 

Any questions or concerns you have regarding the content of the forms and my policies will be addressed at the start of our 1st session. I take time in the beginning of the intake session to explain the policies and ensure you understand them- this is called "Informed Consent."

Individual Form List

If you are a couple or family coming for an intake, all paperwork needs to be completed by all members of the family/couple individually. Only those coming in for couples therapy will need to complete the form entitled "Couples Therapy Informed Consent." 

I encourage all new clients to spend time on the "Client Self Assessment" and to be as detailed as you can be. However we can also get into those details in our first several sessions. I do a thorough assessment on all new clients, including a Comprehensive Clinical Assessment and do want to know your history.

To reiterate, if you would like to get into those details more in sessions and/or answering all or some of the questions is triggering or upsetting in some way, we can do that in session; many clients need to go slowly with this. I do not want you feeling overwhelmed or triggered when doing the Self-Assessment, or when I'm getting history from you in the first several sessions. Some people do not get into the specifics or details of their history until later, once more trust and safety is developed, and you are better able to share those, or even think about them, without getting highly triggered. Please don't push through the clinical history if at any time you are feeling it's really triggering or activating you. Fill out what you can in there, and leave some sections blank if you need.

We will be going over your Self-Assessment in the first few sessions. With some clients we don't go over this much at all until more safety is developed and there are skills in place to manage any discomfort that arises. I do not want to re-traumatize you!

Some new clients prefer to have fewer appointments in the beginning around history-taking/ assessment, and want to move right into "the work" as fast as possible; if this is you, please complete the Self-Assessment as detailed as you are able, and make sure you are not skipping sections.

New clients can expect more time in assessment, history-gathering and arriving at an initial diagnosis, than what you may have been used to with other private practice therapists, depending on who you have seen. I like for my new clients to know that this is something to expect when starting to work with me. It allows me to get a good idea of what is going on, what areas you need and want help in, and to see if I'm a good fit for that, as well as to start creating an effective and individualized treatment plan. During this time we are also getting a feel for one another, and working on building safety. This is also the first phase of the treatments I do, including EMDR.
